Clinical Case: Esthetic Rehabilitation of a Maxillary Central Incisor with an Implant

Problem

Significant destruction of the clinical crown of tooth 21 and compromised smile aesthetics.

Solution

Clinical examination, radiographic assessment, and CBCT revealed chronic apical periodontitis of tooth 21. Restorative treatment was not possible due to subgingival destruction and an inadequate ferrule.

  • Atraumatic extraction of tooth 21 with preservation of the bone volume and gingival contour.
  • Immediate implant placement.
  • Immediate placement of a fixed provisional restoration to preserve the interdental papilla and shape a natural gingival contour.
  • Following osseointegration, the soft tissues were refined with a provisional crown, followed by placement of the final highly esthetic implant-supported crown.
  • The maxillary central incisor was successfully restored in the esthetic zone. The crown accurately reproduces the color, translucency, and surface texture of the adjacent tooth 11, while the natural gingival contour ensures harmonious pink and white aesthetics.
