1. General Provisions
These Terms of Use and Privacy Policy (“Terms”) govern the use of the website and mobile application operated by INNOVATION FAMILY POLYCLINIC L.L.C (the “Clinic”), registered in Dubai, United Arab Emirates.
By accessing or using the website or mobile application, the user confirms that they have read, understood, and agreed to these Terms in full. If you do not agree with any part of these Terms, you should discontinue use of the service.
2. Limitation of Liability
The information presented on this website is provided for general informational purposes only and does not constitute medical advice.
The Clinic assumes no responsibility for any errors, interruptions, or consequences arising from the user’s reliance on information published on the website without consulting a qualified medical professional.
All medical services are provided exclusively in person at the Clinic in accordance with the license issued by the Dubai Health Authority (DHA).
3. Online Services
The website enables users to book appointments, access information about the Clinic’s doctors and services, and download the mobile application.
Appointment requests submitted online are subject to confirmation by the Clinic and do not constitute a confirmed booking.
The Clinic does not provide telemedicine or online medical consultations through the website or mobile application.
4. Personal Data
4.1. Data Collection.
The website and mobile application (collectively referred to as “we”) collect a limited amount of personal data — namely: full name, contact phone number, age, and email address — solely for the purposes of appointment scheduling, communication with users, and service improvement.
We do not collect Emirates ID, passport information, financial data, or insurance details through online forms.
4.2. Data Processing, Medical Records and Storage.
All personal data is processed in accordance with Federal Decree-Law No. 45 of 2021 — UAE Personal Data Protection Law (PDPL).
The SIMPLEX Patient Mobile Application stores only the minimum patient demographic information required to provide the application’s services, such as patient identification, contact information, and account details.
The application provides secure access to medical information that is maintained within the SIMPLEX HIMES Hospital Information Management System through secure APIs.
Medical records, including but not limited to consultation notes, diagnoses, prescriptions, laboratory results, radiology reports, clinical documents, billing records, and encounter history, are not stored by the mobile application. These records remain part of the healthcare provider’s Electronic Medical Record (EMR) system.
As these records are healthcare records maintained by licensed healthcare providers, they cannot be deleted upon patient request through the mobile application where applicable healthcare laws, regulatory requirements, or medical record retention policies require their preservation.
Personal data is stored on secure systems and is not shared with third parties unless required by applicable law, necessary to provide the requested services, or authorized by the user.
4.3. Data Retention and User Rights.
Users may request deletion of their mobile application account and locally stored application data at any time by contacting swsupport@intuitme.com or through the account deletion option available within the application (if applicable).
Upon verification of the request, we will delete or anonymize the personal information maintained specifically for the mobile application within 30 days, except where retention is required by applicable law.
Where permitted by applicable law, requests relating to correction, access to, or deletion of medical records must be submitted directly to the healthcare provider, which will process such requests in accordance with local healthcare regulations.
Deletion of a mobile application account does not result in the deletion of the patient’s medical records.
Personal information is retained only for as long as necessary to provide the services, comply with legal and regulatory obligations, resolve disputes, and enforce our agreements.
Medical records are retained in accordance with the applicable healthcare regulations and statutory retention requirements of the country in which the healthcare provider operates.
5. Use of Cookies
The website uses cookies to ensure proper functionality and to analyze web traffic.
By continuing to browse or use the website, you consent to the use of cookies. You may disable cookies in your browser settings; however, this may affect the site’s performance or certain features.
6. Intellectual Property
All materials on the website and mobile application — including but not limited to text, images, graphics, and logos — are the property of INNOVATION FAMILY POLYCLINIC L.L.C and are protected under applicable UAE laws.
Any reproduction, distribution, or other use of materials without prior written authorization from the Clinic is strictly prohibited.
7. Amendments
The Clinic reserves the right to amend these Terms at any time without prior notice.
The updated version of the Terms will be published on the website and shall take effect immediately upon publication.
8. Governing Law
These Terms are governed by and construed in accordance with the laws of the United Arab Emirates.
Any disputes arising hereunder shall fall under the exclusive jurisdiction of the competent courts of the Emirate of Dubai.
